Patient Forms
Our Patient Forms page allows both new and returning patients to fill out important paperwork before arriving at the office. Filling out paperwork in advance can reduce your time in the waiting room and allows us to attend to your dental needs more quickly!
Payment Options
We are happy to discuss fees and payment options before treatment begins so that you know exactly what to expect. We have payment plans to suit different budgets, including a no-down-payment option. We also accept most insurance plans, and for your convenience we accept VISA, MasterCard, and Discover.
Fees
In an effort to keep fees down while maintaining the highest level of dental care, we have established these policies:
- If full payment is made at the onset of treatment, we will offer a fee discount. (If you have insurance, we will offer a discount for payment of your portion of the fee that will not be covered by insurance.)
- To fit your individual needs, financial arrangements can be made to extend your payments over a period of months. For your convenience, we accept payment by all major credit cards.
- In cases of serious delinquency, treatment will be discontinued, although we will make every effort to accommodate patients with temporary difficulties.
Dental Insurance
If you have insurance, we will help you to determine the coverage you have available. We ask that you assign your insurance benefit to us. The balance will be arranged for you to pay. Please remember that dental care is provided to you, our patient, and not to an insurance company. Thus, the insurance company is responsible to the patient, and the patient is responsible to the doctor.

Our office takes safety extremely seriously. Our office has built strict
protocols in these times of unknown to get us through the pandemic with
safety being our number one concern. To our surprise the word got out by
our unbelievable patients. Last week we were contacted by NBC news and
